In accordance with Colorado law, I am gifting one or more samples to The Good Lab for testing for no remuneration with the understanding that any excess material will either be returned to me or destroyed.

I understand that information about my product is provided as a service and makes no claims of efficacy or safety.

I understand that The Good Lab will keep my information confidential and will not discuss it with a third party without my consent.

I understand that test results are applicable only for the sample tested and for the specific test conducted.

I agree not to change, alter or reproduce test reports in any way without the expressed consent of The Good Lab. I understand that any reports that have been altered without permission will be considered fraudulent.

I understand that samples received after 6:00pm will be included in the following day's business.

I agree to pay when samples are submitted, prior to sample preparation or testing. I understand that results will not be released until payment has been received.

I understand that The Good Lab will not retain samples beyond 30 days after submission, except for ongoing research and development projects that have been arranged in advance..

I agree that it is my responsibility to maintain the accuracy of this account information and to inform The Good Lab in writing of any changes to the contact information on file.

I further understand that due to many factors outside of The Good Lab's control, results may vary. Variations in results may be due to natural variations in the plant, errors in production process, and/or inaccurate sampling practices.
